Referral Process
We are accepting new patients and we ask for a professional referral to obtain pertinent medical records, labs, diagnosis and medications. Professionals can complete the referral form, found in the link below. Referral sources will submit the referral form for the patient/client and return to our office with diagnosis, medication list and last two session/encounter notes if available. All referrals are screened by our providers to make sure a higher level of care isn't needed.
**Please do not promise patients, on our behalf, we will prescribe or continue certain medications. We are complying with the FDA and are not prescribing Benzodiazepines and Opiates together.
Due to the complications associated with patients with diagnosis of Autism, Aspergers, and IQ less than 70, PDD or any type of developmental delay, we may not be able to accept the referral. If we feel the needs are greater than we can provide, we will make a recommendation.
We will contact the patient with an appointment date and time and notify you if the patient cannot be accepted in our program. Due to the high volume of referrals, please follow up with the patient for their appointment date.
There is a no tolerance policy in place for new patient no-shows and we will not reschedule in the future if the patient does not cancel within 24 hours of the appointment.
